Out-of-Network Provider

To provide the highest level of care without compromise, Dr. Gutierrez has chosen to remain an out-of-network provider. Your insurance policy is a contract between you and the insurance company, and not between the dentist and the insurance company. Dr. Gutierrez and our patients enjoy having the freedom to choose what is best for their care, rather than being dictated by an insurance plan’s coverage.

Our office will still accept your insurance benefits, and, as a courtesy, will file insurance claims on your behalf. We will do our best to manage the paperwork that is needed to seek insurance reimbursement and help you maximize your insurance benefits. Please note:

• Payment is due in full at the time of service.

• Your insurance company will be instructed to send the reimbursement check directly to you, the insured.

• Ultimately, the patient, parent, or legal guardian is fully responsible for payment and for the account.
